INITIATING HAZARDOUS AGENT PROJECTS

Prior to initiating projects involving hazardous agents (ABSL 2 hazards, chemical advisories, and radioactive material) in animal models, a LAMS Facilities Hazardous Agent Usage Notification form must be completed and submitted to the LAMS Hazardous Materials Officer (HMO) at ML 0571. Once the form has been submitted, the LAMS HMO will schedule a hazard assessment meeting with research staff, LAMS management, and appropriate safety office representatives.

The purpose of this meeting is to ensure that all parties are informed of the safety precautions, start and end dates, species and number of animals, PPE requirements, waste and carcass disposal, room entry procedures, appropriate signage, room location, and any special study needs.
